First mRNA cancer vaccine succeeds in trial, but projected $475,000 price raises affordability concerns
Moderna and Merck's personalized mRNA cancer vaccine hit Phase 3 endpoints, but a projected $475,000 price tag has sparked debate about access.
Less than a week later, investment bank William Blair projected that the vaccine could carry a wholesale acquisition cost of $475,000 per patient, about 3.18 million yuan. The estimate, part of a sales forecast model, is not a company pricing announcement. It is unclear whether the figure refers to a single dose or the full course, but the treatment is intended to be used with Keytruda. Adding Keytruda's list price of up to $221,000 for nine cycles, the total drug cost could approach $696,000.
The high cost is rooted in personalized manufacturing. Each patient's tumor is sampled and sequenced; algorithms select up to 34 neoantigens, and a unique mRNA is designed and produced for that individual. According to Jiemian News, Zhang Qilin, a chief physician at Shanghai Public Health Clinical Center, estimated that manufacturing alone exceeds $100,000 per patient, with six to nine weeks needed from sampling to the first injection.
The therapy's development has spanned nearly a decade. Merck paid $200 million upfront in 2016 to collaborate with Moderna. The first patient was dosed in November 2017. A Phase 2b trial showed Intismeran plus Keytruda reduced the relative risk of recurrence or death by 49% and distant metastasis or death by 59% compared with Keytruda alone. The Phase 3 trial, coded INTerpath-001, enrolled 1,137 patients with completely resected high-risk stage IIB-IV melanoma and used a randomized, double-blind design, with one arm receiving Intismeran plus Keytruda and the other placebo plus Keytruda.
AI was involved from design to manufacturing, but Moderna says the system uses machine learning and optimization algorithms rather than large language models. The platform automates steps from sequencing data to drug design and production scheduling. Moderna spent $3.132 billion on R&D in 2025, against a net loss of $2.8 billion.
The companies have not published full Phase 3 data or disclosed a price. Overall survival is still being followed.
